Your name
Names and ages of all students
The general area/neighborhood your pool is located in
Each student's level: Can they put their head underwater? Front and back float? Swim independently? Dive to a certain depth to retrieve a sinking toy? Kick on their back? Do strokes other than freestyle? Tread water? Have they taken lessons before? Are they confident in the water, and do they have any specific fears relating to swim lessons?
Whether you would like to start with a 15-minute trial lesson or a 30-minute full lesson. (15-minute trial lessons are good if you aren't sure of your student's level and/or want to know more about my teaching style before committing to a longer lesson. They are always $20.)
